FAIR Health Atlas

FAIR Health Atlas

FAIR Health Atlas visualizes chronic disease data across all 50 states and Washington, DC, revealing where Americans face the greatest health challenges. We also include relevant social determinants of health that influence the risk factors for these diseases. Explore disease prevalence and risk factor patterns, compare regions and track how health outcomes change over time to support research, policy decisions and community health initiatives.

Access to Information about Health Disparities and Chronic Disease

FAIR Health Atlas aims to give policy makers, nonprofits, researchers and the broader public access to relevant information about health disparities in the United States. Chronic disease prevalence has been generally increasing over time but affects certain regions more intensely. Prevalence among the commercially insured is particularly interesting because they make up most of the population; chronic diseases often first appear during the period of life when people are commercially insured. With FAIR Health Atlas, you can view data as a map, search for locations, switch between state and county view, change chronic disease and year selections, and select from among various social determinants of health.
